Highlighting Resources Across Georgia to Build Healthier Communities

Project Overview

The Community Health Resource Project (CHRP) collaborated with rural and underserved counties in Southeast Georgia to identify and map One Health resources that promoted positive health outcomes. One Health resources aren’t just your doctor and dentist, they are also parks and recreation, healthy food, housing, senior services, education, environmental services, healthcare, daycare, disability services, animal/veterinary services, transportation, social services, public safety, and agricultural services. In addition, the data collected for each partner county helped support workforce development strategies.

Project Design

Prepare, Engage, Report (PER)

  • Preparation Phase: Team members conducted an environmental scan of each county, identified leading health indicators and compiled demographic and geographic data. They also identified organizations promoting positive health based on social determinants like economic stability, education, healthcare access, neighborhood environment, and social context.
  • Engagement Phase: The team sent introductory emails, conducted windshield and walking tours with drop-in conversations, and scheduled formal meetings with community members and leaders to gather information on health assets. They also attended community events, like farmers’ markets, to build trust and relationships. 
  • Reporting Phase: Asset maps and detailed county reports were created. These reports highlighted how counties utilized their resources to meet health needs. Each county partner received their specific report and access to the health asset map. A final project report was also created.

Project Reach

Prepare, Engage, Report (PER)

In the first year, CHRP focused on community outreach and relationship building, which led to six county-specific community asset (resource) maps and reports. At the end of year two, 18 county reports were developed in total using a strengths-based approach to highlight existing resources and services that contributed to positive One Health outcomes.
